What Is a Musculoskeletal (MSK) Ultrasound?
MSK ultrasound provides real-time, dynamic imaging of joints, tendons, and soft tissue — often matching MRI accuracy at a fraction of the cost and wait time.
In brief: An MSK ultrasound uses high-frequency sound waves to produce real-time images of muscles, tendons, ligaments, bursae, and nerves. It is the first-line imaging investigation for most tendon and soft tissue conditions, and can also guide therapeutic injections. At Victoria Medical, same-day MSK ultrasound is available with results provided immediately.
What Does an MSK Ultrasound Assess?
A 2021 systematic review and meta-analysis in the Orthopaedic Journal of Sports Medicine found that ultrasound demonstrated statistically equivalent diagnostic accuracy to MRI for both full-thickness and partial-thickness rotator cuff tears — with a pooled sensitivity of 0.87 and specificity of 0.96 for full-thickness tears.[1] The key advantage of ultrasound over MRI is its dynamic capability: the sonographer can assess tendons and joints in motion, which is not possible with static MRI imaging.
| Condition | What Ultrasound Shows |
|---|---|
| Rotator cuff tears | Full and partial thickness tears of supraspinatus, infraspinatus, subscapularis |
| Tendinopathy | Achilles, patellar, biceps, and other tendon degeneration and thickening |
| Tennis elbow (lateral epicondylitis) | Common extensor tendon pathology at the lateral elbow |
| Plantar fasciitis | Thickening and degeneration of the plantar fascia |
| Bursitis | Subacromial, trochanteric, prepatellar, and other bursae inflammation |
| Ganglion cysts | Fluid-filled cysts arising from joint capsules or tendon sheaths |
| Ligament injuries | Ankle ligament tears, knee collateral ligament injuries |
| Nerve entrapment | Carpal tunnel syndrome, Morton's neuroma, ulnar nerve at elbow |
MSK Ultrasound vs MRI: When to Choose Which
| Feature | Ultrasound | MRI |
|---|---|---|
| Radiation | None | None |
| Cost | Lower (£150–£350) | Higher (£400–£900) |
| Availability | Same-day at most private clinics | Often 1–4 week wait |
| Dynamic assessment | Yes — real-time movement assessment | No — static images only |
| Guided injections | Yes — real-time guidance | Limited |
| Soft tissue detail | Excellent for tendons, bursae, nerves | Superior for bone marrow, cartilage |
| Best for | Tendons, bursae, muscles, nerves, joints | Bone marrow oedema, cartilage, ligaments |

From Our Practice — Dr George Xynopoulos
"For most tendon and soft tissue presentations — shoulder pain, tennis elbow, Achilles tendinopathy — I reach for ultrasound first. It gives me a real-time, dynamic picture that I can correlate directly with the patient's symptoms. I can ask the patient to move the joint while I scan, which often reveals pathology that static imaging would miss. For bone marrow oedema or complex ligament injuries, I will refer for MRI. But for the majority of MSK presentations, ultrasound is faster, cheaper, and equally informative."
Frequently Asked Questions
Is an MSK ultrasound better than an MRI?
For most tendon, bursa, and soft tissue conditions, ultrasound is equivalent to MRI and has the advantage of being real-time, dynamic, and immediately available. A 2021 meta-analysis found no statistically significant difference between ultrasound and MRI for diagnosing rotator cuff tears. MRI is preferred for bone marrow pathology, cartilage assessment, and complex ligament injuries.
Do I need a GP referral for a private MSK ultrasound?
No. At Victoria Medical you can self-refer for a private MSK ultrasound without a GP referral. You can book directly online or by phone.
Can ultrasound guide injections?
Yes. Ultrasound-guided injections allow the clinician to place the needle precisely into a bursa, tendon sheath, or joint under real-time visualisation, improving accuracy and reducing the risk of inadvertent injection into surrounding structures.
How long does an MSK ultrasound take?
A focused MSK ultrasound of a single joint or tendon typically takes 15–25 minutes. If multiple areas are being assessed, the scan may take longer.
What should I wear to my MSK ultrasound?
Wear loose, comfortable clothing that allows easy access to the area being scanned. For shoulder or elbow scans, a sleeveless top or short-sleeved shirt is ideal. For knee or ankle scans, shorts or trousers that can be rolled up are helpful.
