How We Test for B12 Deficiency
Diagnosis begins with a serum B12 level. If borderline (200β300 ng/L), I request methylmalonic acid (MMA) and homocysteine as second-line biomarkers β both rise when functional B12 is insufficient. A full blood count may show macrocytic anaemia, though neurological symptoms can present before any haematological changes appear.[5]

How quickly do B12 injections work?
Most patients notice improved energy within 48β72 hours of the first injection. Neurological symptoms may take several weeks to months to improve, depending on how long the deficiency has been present.
Can I just take oral B12 supplements?
If your deficiency is caused by dietary insufficiency (vegan or vegetarian diet), oral supplements are usually effective. However, if you have pernicious anaemia or malabsorption, injections are necessary because oral B12 cannot be adequately absorbed.
How often will I need injections?

What blood tests do I need?
I typically request serum B12, full blood count, and folate. If B12 is borderline, I add methylmalonic acid and homocysteine. If pernicious anaemia is suspected, intrinsic factor antibodies are included.
Can B12 deficiency cause permanent damage?
Yes β prolonged untreated deficiency can cause irreversible damage to the spinal cord and peripheral nerves (subacute combined degeneration). This is why early diagnosis and treatment are essential.
